About Provider Connect, LLC
Provider Connect was started in October of 2016 as a collaboration of Anne "Libby" Demerit, PT and Jennifer Haefner, PTA. After working together for over 15 years in various settings, the two decided to start a company that would benefit both home health agencies and therapists. In June of 2020, Libby left Provider Connect to pursue other interests and Jennifer has continued the business herself.

Provider Connect only networks with the area's best therapists to provide quality treatments to patients and excellent communication to the entire care team. It is the company's mission to assist home health agencies with staffing issues while at the same time assisting therapists to reach their financial and career goals.
About the Owner
Jennifer Haefner, PTA
Jen graduated from Sanford Brown and became licensed in December of 1998. In January of 1999, she began working as a PTA in the neurorehab department of BJC. Her career focused on stroke and brain injury rehab during the entire 8 years she worked with BJC, which eventually became The Rehab Institute of St. Louis. "I absolutely loved working at TRISL. We saw many different diagnoses and people from many different backgrounds. This is when neurorehab became my passion." From there, she developed an aquatics program for an outpatient clinic and managed the clinic for 5 years before working PRN with several SNF and home health companies in order to spend more time with her family. Jen decided to partner with Libby in order to be able to make her own hours and help other therapists like herself.

Jen has been married to Bob since 2000, has two children and lives in South St. Louis City. In her spare time, she enjoys playing roller derby and spending time with friends & family.
